About CM Care Management
CM Care Management provides care management services for seniors and their families. Our services include assessment, advocacy, crisis management, placement, information and referral, money management, and on-going care management services.

Cheryl Oeltjenbruns has a master's degree in social work and has worked in the home health field in Delta and Montrose counties for over 15 years. She is a member of the National Association of Professional Geriatric Care Managers and the National Association of Social Workers.

Michelle Hilton has a master's degree in gerontology and over 10 years experience working with the senior population in Delta, Montrose, Gunnison, Hinsdale, Ouray, and San Miguel Counties. She is active in the community through a variety of venues including community boards and educational/artistic capacities.
